Job Type/Schedule This position is full-time and works approximately 40 hours per week. Department Name/Job Location This position is in the Pediatrics Diagnostic Center. This position is for the Medical School Campus. Essential Functions POSITION SUMMARY: The Case Manager for the COACH (Creating Options and Choosing Health) provides medical case management for youth who have recently entered foster care and obtain services through the COACH program at The SPOT, as a member of the multidisciplinary care team. Once youth are seen they can remain in the COACH program until age 25. The SPOT Youth Center provides free medical, behavioral health, and prevention services to a diverse population of youth ages 13-24 years old. A positive youth development approach and shared youth decision-making are incorporated into all environments served. The COACH Case Manager will work in coordination with the youth, Children’s Division case managers (staff), and foster parents to develop and implement care plans to address medical, social, mental health, prevention and educational/vocational needs. The Case Manager will provide community resource referral and coordination, crisis intervention and supportive counseling, and monitor and coordinate youth medical needs. This position serves youth with diverse backgrounds of race, ethnicity, sexual orientation and gender identity and comfort and expertise in cultural congruency will be important and requires a positive, non-judgmental attitude and a willingness to work with young people between the ages of 13-24. The COACH Case Manager will abide by the same strict standard of patient confidentiality that applies to all Washington University staff. PRIMARY DUTIES AND RESPONSIBILITIES: The COACH Case Manager will provide case management services to clients including the following expectations: 1. Prepare for weekly COACH clinics and ensure that all necessary materials are in place for each clinic day. 2. Arrange and coordinate service referrals for the youth/family as needed; provide ongoing monitoring of the care plan objectives; and work with the youth and families to identify and remove barriers to care. Refer clients/families to public benefits programs as eligible. 3. Provide regular contact with enrolled clients, attend family support team meetings as able, and document all contact and service coordination activities in electronic record systems as required by program guidelines. 4. Assist youth with securing needed medical follow up/services and monitor kept appointments. 5. Conduct behavioral health assessments with the client in order to develop a plan of care to meet identified needs. 6. Meet with clients in the community to assist in facilitation of any needed medical services and provide supportive case management services not available to client through foster care placement. 7. Maintain current knowledge of and incorporate evidence-based interventions for youth, and youth in foster care. Routinely interact and network with community providers, clinicians, and others involved in youth service delivery. Advocate for improved services for youth. 8. Work in coordination with SPOT staff to develop and implement a range of youth center programming that will address primary issues impacting the lives of youth. Co-facilitate needed programs and/or work with community partners to develop/provide needed services. 9. Utilize an empowerment/developmentally appropriate framework to assure that youth are active partners in all aspects of the services received at the center. 10. Perform indirect, non-clinical tasks related to the job, including compilation of data, participation in required training, and attendance at staff meetings and community meetings as assigned. 11. Maintain confidentiality and privacy of clients per HIPAA guidelines. Additional Functions: 1. Work collaboratively with Children’s Division staff and foster parents. 2. Work collaboratively within the SPOT multidisciplinary team, Project ARK and all community partners. 3. Provide case management services; including coordinating services with medical and social service teams. 4. Attend meetings as scheduled to coordinate, advocate and plan for youth services. 5. Perform indirect, non-clinical tasks related to the job, including regular attendance at staff meetings and timely compilation of client data upon request. Required Qualifications Equivalent of a bachelor’s degree in a health related field, such as social work, counseling, sociology, or psychology. Applicants with bachelor’s degrees in other fields may be considered with sufficient case management experience. Preferred Qualifications * Equivalent of a master's degree in a health related field, such as social work, counseling, sociology, or psychology. * Provision of case management and/or prevention education services. * Knowledge of St Louis City and St Louis County Children’s Division and needs of youth in foster care. * Knowledge of community resources. * Computer skills. Salary Range The hiring range for this position is $18.81 - $23.52 per hour.
