Job DescriptionWe are hiring a Care Coordinator (PCMH) to work with our patient population for Primary Care. This is a full-time position that will sit at the Smithville location. The work hours are 8 a.m. to 4:30 p.m Monday through Friday.The Care Coordinator Patient Centered Medical Home is responsible for the application of extensive knowledge in clinical performance improvement activities.This individual will be responsible for driving engagement strategies for improving physician and staff patient care practices based on the Patient Centered Medical Home model clinical guidelines, best practices, internal and external benchmarks and claims based data.The PCMH Coordinator supports and ensures the overall success of the PCMH model by collaboratively working with patients physicians practice teams and health plans to select guide and direct patient care while integrating key features of the medical home.Important to Note:Dual License Required - MO RN License and KS RN LicenseBachelor's Degree RequiredBenefits:Health, Vision and DentalLeave of Absence, PTO, Extended Sick Leave, and various Welfare plansFlexible Spending AccountsHealth Savings AccountsRetirement OptionsEducation ReimbursementJob RequirementsApplicable Experience:Less than 1 yearRegistered Nurse - VariousBachelor's Degree (Required)Job Details Full timeDay (United States of America)"The best place to get care.
