CareSTL Health Nurse Case Manager - Registered Nurse in Saint Louis, Missouri

CareSTL Healthseeks multiple full-time Registered Nurse Case Managersto develop, implement, and evaluate individualized patient care plans. They act as social workers, advocate patient welfare, and serve as a liaison between patients, their families, and healthcare providers.ESSENTIAL FUNCTIONS: The following information is considered the definition of essential functions but does not restrict the tasks that may be assigned. The RegisteredNurse Case Managermay be reassigned duties and responsibilities at any time due to reasonable accommodation or other staffing reasons.Review treatment goals and resources to evaluate patient and determine level of care; when the patient needs further, home planning, action plans and complete an expanded assessment of patient at time of initial/new clinic encounterCommunicates with physicians and other agencies and service providers to develop working relationship that facilitate case coordination and information sharing, while also facilitating investigations as well as medical and treatment referralsInterpret medical documents while also helping patients understand their conditions and treatment options in addition to the evaluation of patient satisfaction and quality of care providedCoordinate the integration of social services/case management function while ensuring that patient tests are appropriate and necessary, while maintaining appropriate costParticipate in the development and implementation of patient care policies and protocols, while promoting effective and efficient utilization of clinical resourcesAdditional Functions:Refer cases where patients and/or family would benefit from counseling and help patient and family make informed decisionsProvide education and literature about diagnosisActively demonstrate a working knowledge of nursing theories, techniques, principles and practice in order to holistically care for the patientEducation:Graduate of an accredited School of NursingValid registration as a Registered Professional Nurse in the State of MissouriCurrent AHA Healthcare Provider training (BCLS)Experience: 1-2 years experience working in Community Health Center/or Hospital settingTransportation Requirement: Must have reliable transportationA Successful Candidate:Knowledge of information and techniques in providing excellent patient careKnowledge of administrative and office procedures and systems such as electronic transmission via facsimile and/or internetGood computer skills including the use of office procedures and terminologyGood self motivational and organizational skillsAbility to maintain confidentialityAbility to work all health center sitesUnderstanding of the complexities of integrating servicesKnowledge of data management through use of ITFor more information, or to apply now, you must go to the website below.
