  • 2+ years of experience as a LPN/RN
  • Ability to pass an extensive criminal background check
  • Vehicle, auto insurance, and valid driver's license
  • Ability to lift 55 lbs

RN / LPN Responsibilities:

  • One on one skilled care / nursing care in a patient’s home
  • Provide nursing services following the direction of collaborating physicians
  • Partner with patients and their families, therapists, physicians, and other members of the patient care team to ensure the highest level of care is provided each day.
  • Monitor, document, and assess the progress of the patient’s health and condition while providing updates on their treatment plan
  • Demonstrates competent technical skills and delivers high-quality patient-focused treatment
  • Understand and adheres to established policies procedures
  • Implements the nursing care plan for each patient
  • Experience with Trachs, Feeding Tubes, Ventilators, Wound Care, Medication Management, Foley Care Insertion, Ostomy Care
  • IV certification and/or experience preferred but not required
  • Ability to work up to 12 hour visits (7a-7p or 7p-7a), and/or short visits, based on client needs
  • Days, Nights, Weekend Rotations available RN / LPN Requirements:

  • High School diploma or GED

  • Current state-approved LPN or RN license preferred
  • Experience utilizing assisted devices such as a Hoyer Lift, Gate Belt, Sit to Stand, etc.
  • High level of patience and understanding of difficult behaviors
  • Ability to develop and implement various activities to help support your client in their day-to-day activities
  • Commitment to Education and learning how to continually increase your skillsets
  • Compassionate, mature, sympathetic and professional demeanor
  • Willingness to work within a strong team
  • Excellent communication skills
  • Ability to observe and identify non-verbal cues and client needs Caregiver Responsibilities:

  • Create and maintain meaningful relationships with patients and their patient’s families

  • Bring Sunshine Smiles to each and every client, family member, and care team!
  • Provides personal care such as bathing, dressing
  • Cook and serve meals and assists with eating
  • Assists and lifts patients from bed to chair and/or wheelchair
  • Carefully observes and maintains care records about conditions and treatment plans related to the client’s changing needs
  • Light housekeeping
  • Outstanding level of communication with everyone involved in your care team
  • Provides transportation and/or accompany clients to doctor visits and other personal appointments
  • Assist patients with mobility and physical therapies and exercises
  • Ability to take the initiative and work to seek positive changes
