This position is located in the Health Information Management (HIM) section at the John J. Pershing VA Medical Center. Health Information Technicians (HIT) review, analyze, abstract, compile, maintain, and extract information from the health record. They also check for completeness and accuracy of the health record, and monitor and report for regulatory compliance.

Incumbent is responsible for performing quantitative and qualitative reviews of health record documentation.
Incumbent is responsible for ensuring that all patient care data entered in VISTA and/or Computerized Patient Record System (CPRS) is accurate, timely, and completed. Adheres to established documentation requirements as outlined by The Joint Commission regulations, Veterans Health Administration (VHA) guidelines, as well as medical-legal requirements.
Reviews record for adherence with CMS guidelines related to student, resident supervision and attending physician presence and documentation.
Compiles reports to insure provider specific and service compliance. Maintains a control system to ensure completion of all inpatient and outpatient records in accordance with VA, The Joint Commission, CMS and other regulatory agencies standards.
Performs the quantitative and qualitative analysis on both computer and paper base records.* Identifies health record deficiencies, tracks deficiencies and ensures that the appropriate individual completes their deficiencies.
Assists physicians with the completion of delinquent/incomplete health records and serves as the expert resource for clinical staff regarding requirement for complete documentation.
Assesses the paper health record and electronic Computerized Patient Record System (CPRS) for completeness and notes deficiencies.
Tracks electronic signatures and documents deficiencies and communicates these deficiencies to clinical staff for correction.
Prepares complex weekly and monthly reports noting the status of all incomplete/delinquent records and compliance reports and submits the reports to the appropriate administrative and clinical staff.
Develops and compiles written reports trending provider specific, service, and hospital compliance reports regarding admission subsequent and discharge notes.

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 03/01/2019. Basic Requirements:

English Language Proficiency. MRT's must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Experience and Education
(1) Experience; One year of experience that indicates knowledge of medical terminology and general understanding of the health record. Six months of the required one year of experience must have provided the knowledge, skills and abilities (KSA's) needed to perform MRT work. OR, (2) Education; Two years above high school with a minimum of 12 semester hours directly related to MRT work (e.g., courses in medical terminology, anatomy & physiology, and introduction to health records). OR, (3) Experience/Education Combination; Equivalent combinations of experience and education are qualifying.The following educational/training substitutions are appropriate for combining education and experience: (a) Six months of experience that indicates knowledge of medical terminology and general understanding of the health record and one year above high school with a minimum of 6 semester hours of health information technology courses. (b) Successful completion of a course for medical technicians, hospital corpsmen, medical service specialists, or hospital training obtained in a training program given by the Armed Forces or the U.SMaritime Service under close medical and professional supervision may be substituted on a month-for month basis for up to six months of experience provided the training program included courses in anatomy, physiology, and medical record techniques and procedures. Also requires six additional months of experience that indicates knowledge of medical terminology and general understanding of the health record.
Grade Determinations:GS-7 MRT (HIT) Full Performance Level
(a) Experience. One year of experience comparable to the next lower level GS-6. (b) Demonstrated KSA's. In addition to the experience above, the candidate must demonstrate the following KSA's: * * Ability to judge whether medical records contain sufficient information for regulatory requirements; are acceptable as legal documents; are adequate for continuity of patient care; and support education and research needs. This includes the ability to take appropriate actions if medical record contents are not adequate, accurate, timely, and/or reliable; * Skill in using appropriate software and data applications in order to analyze and record documentation trends; * Skill in communicating with individuals or groups who may be uncooperative, or skeptical, in order to obtain the desired effect, such as gaining compliance with established policies and regulations; * Ability to manage priorities and coordinate work in order to complete duties in an accurate and timely fashion. This includes the ability to follow-up on pending issues and an understanding of the impact of incomplete work; * Ability to work independently to accomplish a variety of responsibilities including setting priorities, organizing and adjusting workflows, and utilizing problem-solving techniques in the accomplishment of work; and * Knowledge of qualitative and/or quantitative methods for assessment and improvement of medical record functions. (c) Assignment. This is considered the full performance level for HIT's. HIT's review, analyze, abstract, compile, maintain, and extract information from the health record. Check for completeness and accuracy of the health record, and monitor and report for regulatory compliance.
The full performance level of this vacancy is GS-7. The actual grade at which an applicant may be selected for this vacancy is GS-7 only.
GS-6 MRT (HIT) Developmental Level 2 -
Demonstrated KSA's. * * Ability to apply knowledge of medical terminology and human anatomy to fully understand the content of a patient record; * Ability to navigate efficiently through the paper and/or electronic medical record to locate needed information;Skill in interpreting and adapting health information guidelines that are not completely applicable to the work or have gaps in specificity, and ability to use judgment in completing assignments with incomplete or inadequate guidelines; * Skill in interpreting and adapting health information guidelines and ability to use judgment in completing assignments; and * Knowledge of all components of the health record and ability to analyze and abstract pertinent information to meet Joint Commission and/or regulatory health record documentation requirements. Physical Requirements:See VA Directive and Handbook 5019. The duties and responsibilities of the position require some mobility between various Clinic areas in separate building location sites, but generally the job is sedentary with some standing, stooping, reaching, and lifting up to 20 pounds. Incumbent is required to work with computer equipment.
