Various shifts, varied clinic locations, Full time, PRN available.i Se company website for details.Promotes customer satisfaction through direct patient care competently and professionallyAdheres to the patient focused plan of care, acts as a change agent, presents a professional approach and appearance, a friendly smile, pleasant tone of voice, operates as an active part of the team, uses constructive problem solving, maintains confidentiality, promotes a positive approach to all tasks, refrains from criticism of co-workers, the Home Health, Palliative Care and Hospice Department, Hospital, patients and patient familiesProvides safe and efficient patient careCares for the patient in their home setting under the direction of a physician, takes vital signs, does full systems assessment, performs venipunctures, and demonstrates the knowledge and skills necessary to provide care to all age groupsParticipates in skin care management by providing prescribed wound care, evaluating the appropriateness of the plan of care and reports changes to RN for interventions/care planningDemonstrates and possesses the ability to understand palliative philosophy of hospice careAssess date reflective of the patient's status and interpret the appropriate information needed to identify each patient requirements relative to age specific needs and to provide the care needed as described in Home Health departments Policy and ProceduresAttends case conferences and Interdisciplinary Team meetings for Home Health and HospiceProvides for safety and psychosocial needs of patientsVerifies that patient environment is safeReinforces patient/caregiver educational effortsReports to the RN any medication or care-related changesSuccessfully completes all competencies related to jobAttends staff meetings and in-services (inter and intra-departmental work teams)Maintains a safety-first care approach for patient/caregiver, self, and all members of the HH care teamDelivers high quality service and help control costs in a safe environmentEnsures patient safety, uses supplies efficiently, uses equipment properly, exhibits awareness of patient's rights and respects them at all timesResponds to patient requests promptly, reports to work when scheduled and on timei
