The Community Support Specialist will float and provide supportive services for clients who are experiencing problems as a result of mental illness. Services include triage, risk assessments, ongoing assessment of psychosocial needs, discharge planning, information and referral, client and family education, coordination of services with managed care contacts and/or other service providers. The community support specialist functions as a member of an interdisciplinary team for client care and works effectively in a multicultural setting. The community support specialist works under the regular supervision and direction of a QMHP.
Bachelor's degree in Social Work, Psychology, or other related Human Service field approved by the State of Missouri.
Must have good verbal and written communication skill, good leadership skills, ability to evaluate program out comes, and the ability to work with multicultural staff and clientele.
