Chronic Care Management Supervising Physician

Collaborating Physician

  • Collaborating relationship with assigned staff, including performance management activities, staff development, and review of clinical performance through chart reviews and participation in clinical rounds

  • Participate in an interdisciplinary (IDT) approach to care, physician, NP, RN and LPN, LCSW and chaplain support. This includes regular communication and coordination within the Alegis IDT.

  • Works to achieve quality medical care and outcomes for patients, in a cost-effective manner.

  • Improves clinical outcomes through interactions with providers, using education and excellent written and verbal communication skills.

  • Provides clinical oversight for new providers until such a time as oversight can be decreased with a minimum of 10% review of records or the state minimum

  • Serves as a mentor or coach to nurse practitioners in ongoing quality and performance improvement processes.

  • Improves provider relations through direct communication, knowledge of appropriate evidence-based clinical information, and the fostering of positive collegial relationships.

  • Addresses customer service issues with mentoring and support from leadership staff.

  • Investigates and responds to client or stakeholder questions to assist in resolving issues with mentoring and support from leadership staff.

  • Achieves customer satisfaction and regulatory/accreditation agency compliance goals by assuring timely and complete documentation, including documentation of appropriate clinical measures and metrics.

  • Provides clinical insight and management support to other functional areas and matrix partners as needed or directed.

  • Occasional interaction with local physicians to further collaborative relationship and/or potential participation/interaction with our health plan partners

Qualifications

  • Board-certified physician in Internal Medicine or Family Practice and with a current unrestricted medical license. Experience in geriatrics or palliative care/hospice preferred

  • Exhibits ethical and professional behavior.

  • Minimum of 5 years of clinical practice experience beyond internship, residency, or fellowship training.

  • Preferred: Prior physician leadership positions.

  • Preferred: Experience in medical and case management, knowledge of HEDIS and Stars measures and managed care experience.

  • Computer Competency: Microsoft Office - Word, Powerpoint, Excel competency, EMR utilization, on line medical research.

  • Ability to work within a changing business environment and balance patient advocacy with business needs.

  • Experience with managing multiple projects in a fast paced matrix environment.

  • Demonstrated ability to educate colleagues and staff members.

  • Successful experience with change management.

  • Demonstration of strong and effective teamwork, negotiation, conflict management, decision-making and problem-solving skills.

  • Successful ability to assess complex issues, to determine and implement their solutions, and to resolve problems.

  • Success in carrying out and maintaining cooperative, successful relations with diverse internal and external stakeholders.

  • Demonstrated sensitivity to culturally diverse situations, participants and customers
