OverviewA dynamic position that is responsible for accepting Hospice, Home Health and Home Infusion referrals in the acute care setting , physician clinic setting, and other selected venues such as indentified nursing homes and rehabilitation centers in the St. Louis and Washington areas. Maintains an active presence in these settings and delivers excellent customer service to discharge planners, medical social workers, physicians, and future patients. Assists in the facilitation of timely discharges and support the system initiative for continuity of care within the Company Health System. ResponsibilitiesHospital based liaison will coordinate all referrals from the in-patient setting.Responsibilites include:a. Manages all referrals for Hospice/Home services regardless of the patients provider choice.b. Follows all Company Hospital expectations and guidelines to acheive discharge documentation.c. Acts as a liaison with hospital/clinic staff to acquire proper initiation of services.d.Communicates with physicians and other hospital professionals to assist in the health care plan anddetermine the appropriate venue of care.e. Establishs and maintains effective working relationships within Company Hospital communities.f. Documents all progress and interactions into the EMR prior to the end of work day.g. Provides to the receiving Hospice/ Home Health or Home Infusion program all pertinent demographic andclinical information as requested.Community based liaison will coordinate referral from non-acute Company settings and other selected venues such as identified nursing homes, rehabilitation centers, established by Company Home Services management.Responsibilities include:a. Requests and acquire pertinent clinical information to determine services.b. Completes accurate documentation utilizing electronic medical recordsc. Communicates with physicians, nursing homes, other facilities and families to coordinate services.d. Assists in tracking patients who are in alternate settings to assure return to our programs upon discharge.All Liaisons representing Company Home Services.Expectations include:a. Individually meet and interact with patients and caregivers, providing thorough information about the benefits and services offered by Company Hospice and Home Health.b. Provides to Company Hospice/ Home HealthServices the defined payer source provided by the referralsource, patient and/or caregiver.c. Follows current process and procedure expectations of Company Hospice/Home Health and Company HomeInfusion in regards to communication hand off, documentation, provision of start of care date, disciplinesand initial orders.d Compassion and professionalism go hand-in-hand with us, along with exceptional quality care.
