Boone Hospital Home Care brings the excellence of Boone Hospital Center directly to patients' homes. Patients are sometimes discharged from the hospital, or other facility, while still needing close medical supervision. Other times, a doctor may decide that a patient needs assistance but may not need to come to the hospital. This is where Boone Hospital Home Care steps in. Our caregivers are able to schedule home visits 24 hours a day, seven days a week with around-the-clock, on-call nursing supervision. Boone Hospital Home Care professionals also provide in-depth patient and caregiver materials, patient assessments and insurance verification assistance.

Boone Hospital Home Care provides home care and hospice services to patients 24 hours per day, seven days per week. Home care visits are provided for patients with a skilled medical need and are intermittent in nature. Hospice services are provided for patients diagnosed with a terminal illness. Services include care from the following disciplines: nursing, therapy, aide, social work, chaplain and dietician. In addition to skilled medical care Boone Hospital Home Care professionals provide in-depth patient and caregiver education, patient assessments and insurance verification assistance. Boone Hospital Home Care handles of the financial, regulatory, planning, billing and collection functions for the agency.(Separate corporation from Boone Hospital Center)

Role Purpose

Manages Home Care Divisions(home care & hospice), in collaboration with Director of Home Care Services to ensure quality of care, patient safety, appropriate case management and adherence to applicable laws and regulations. Provides leadership and direction in accordance with system and agency goals and objectives. Responsible to speak to regulatory agencies on the behalf of nursing.

Responsibilities

  • Manages individual(s) including but not limited to: hires, trains, assigns work, manages & evaluates performance, conducts professional development plans. Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership.

  • Manages and coordinates patient care to ensure patients are safe and care and treatment is administered in accordance with the plan of care for both hospice and home health services.

  • Manages all clinical and intake staff inclusive of all disciplines.

  • Manages hospice interdisciplinary team meetings and patient review.

  • Collaborates with Leadership to maintain standard regulatory requirements and applicable laws for all divisions of the agency.
