Position Located at Company Rehab Hospital in Chesterfield/St. Louis, MO Our state-of-the-art facility was designed specifically for patients requiring in-patient rehabilitation services and offers programs that range from single therapies to comprehensive, multidisciplinary programs. Company Rehabilitation Hospital provides rehabilitative care to patients and families utilizing a team approach. Our Treatment Team includes experienced Rehabilitation Nurses who provide excellent service and around-the-clock compassionate nursing care in an exceptional work environment. OverviewFacilitates positive changes in patient's basic functional ability and achieves improved independent functioning. Attempts to provide a positive hospital experience in which patients are given opportunities for growth, learning, and mastering the environment through therapy, education, and recreation. Reporting supervisor: Rehabilitation Manager. ResponsibilitiesSafety/Risk Management:Provides for safety of patients with all therapeutic activities.Coordination of Services:a. Coordinates special events with volunteers, organizations, entertainers, etc.b. Participates in quality improvement/program evaluation activities.c. Assists with analyzing outcome data and providing suggestions for action.d. Organizes out-trips effectively, making appropriate community contacts, coordinating patient schedules, and vehicle schedules.Complex Aspects:a. Serves as a resource person.b. Provides appropriate orientation to new staff.c. Identifies new or updates programs inclusive of determining appropriate policies and procedures, training staff, and monitoring program effectiveness.d. Acts as a clinical instructor for students.e. Assists in departmental or special projects as needed.Primary Aspects:a. Orients patients and/or families to services and evaluates patient s needs.b. Develops patient goals and treatment plan specific to needs.c. Facilitates appropriate group and individual treatment in accordance with goals.d. Reassess patient goals to reflect patient changes.e. Procures supplies and equipment within budget requirements.f. Utilizes support staff and volunteers appropriately.Communications Management:a. Completes documentation within appropriate time limit, i.e. weekly team conference reports, daily treatment notes, etc.b.
