Patient Care Tech (PCT) Aspect of this Role
Listens well and communicates clearly and tactfully
Actively participates in purposeful rounding to include 5 Ps +1 (Pain, Potty, Position, Presence, and Possessions + Plan of Care) and recognizes that rounds will happen with our patients, not on our patients
Follows infection prevention guidelines
Obtains and accurately documents vital signs and Intake and Output, daily weight, specimen collections, and ambulation for all patients when applicable
Responds to patient needs and requests in a timely manner with courtesy
Answers all call light “phone” calls in a timely manner following the patient experience initiatives Reports any abnormal or unexpected findings to the nurse caring for the patient
Performs or assists patients with activities of daily living to include meals, ambulating, personal hygiene, dressing patient, changing bed linens, assisting with toileting, and turning/repositioning
Transports patients to/from other departments throughout the organization
Assures patients remain on telemetry and notifies Banked Monitors when patient is coming off telemetry or leaving the floor for any reason
Documents all care provided accurately and in a timely manner
Follows patient experience initiatives including the “no pass zone and commit- to –sit.
Conducts handoff with oncoming staff at the bedside
Functions as a Patient Safety Attendant as needed with appropriate training
Performs other duties as assigned
Unit Secretary (US) Aspect of this Role
Answers all call light “phone” calls in a timely manner following the patient experience initiatives
Accountable for Call light assignments
Sets the tone on the unit with positive attitude and extraordinary communication with patients, families, and staff
Greets and welcomes every patient and visitor to the unit
Orders supplies – dietary/floor stock/office supplies
Recognizes rounding will happen with our patients, not on our patients.
Pages physicians and staff and arrange physician consults as ordered.
Schedules patient follow up appointments
Enters data in the computer and the patient’s medical record
Files patient documents in the yellow folders and sends documents for scanning to the electronic medical record
Assists with orientation of new employees
Assists charge nurses with patient bed assignments
Assists Manager with compliance checklists
Has working knowledge of equipment and procedures to the unit they are assigned.
Stocks patient rooms and/or desk area
Provides patient list for leadership rounding as asked
Performs other duties as assigned
Become
High school graduation or equivalent, formal or on the job training
Current Basic Life Support completion (CPR)
Current CNA or PCT, attended an approved UAP training course, or has documented work experience as a CNA, EMT or surgical tech in the past three years as noted in state regulations
Must be able to read, write, and speak English
