CV Service Line Administration Full-Time



Night ShiftThe APRN is responsible for quality patient care, driving quality patient outcomes, facilitating patient flow and collaborating with all healthcare providers to achieve the highest level of patient outcomes. APRN's may engage in activities that normally constitute the practice of nursing without credentialing, provided they have preparation, knowledge, and experience necessary to properly execute the practice. Medical diagnostic, therapeutic and management functions will be instituted under medical supervision using the guidelines developed for use in the Hospitalist Service

Clinical Practice

  • Obtains patient histories on cardiac surgery

  • Performs physical assessments pertinent to patient's chief complaint

  • Evaluates assessment data and established diagnoses

  • Differentiates patient care situations, which require immediate and non-immediate actions and responds accordingly

  • Orders and evaluates appropriate diagnostic studies

  • Performs and monitors therapeutic procedures

  • Assesses and/or manages follow-up plans or initiates referral as indicated

  • Records and documents assessment data, interventions, results, and patient outcomes

  • Educates patients and/or families in order to promote wellness, prevent health problems, maintain current health and intervene in acute or chronic illness

  • Documents discharge teaching and patient instructions for follow-up

  • Performs appropriate referrals for patient consultation for acute problems or to appropriate specialty clinics for follow-up

  • Performs other duties as assigned


  • Collaborates with Pre Anesthesia Testing to begin education and ensure that all required documents are complete and on the patient records to facilitate patient flow for CV surgery.

  • Performs history and physical exams (H&P’s) capturing all patient comorbidities, accurately representing patient acuity and allowing preparation of a safe plan for the patient. Performs accurate documentation from the H&P to ongoing patient notes to ensure that acuity is captured and quality measures (STS, heart failure and discharge requirements) are met at 100%

  • Collaborates with all Perioperative leadership and staff to ensure efficient patient flow

  • Drives relationship with anesthesia providers to achieve quality patient care and efficient patient flow throughout the patient course including pre, intra and post-operative phases of the patient stay

  • Reviews CV surgery discharge plans to ensure all quality metrics are achieved at 100%.

  • Ensures accurate admission and discharge medication reconciliation

  • Prepares patients for discharge to include required discharge plans are complete (home oxygen set up, required DME arrangement) prior to time of discharge

  • Collaborates with Respiratory Therapy, Case Management and Social Work to ensure the above

  • Ensures that all patients are discharged with post discharge appointments with PCPs, CV surgeons and other providers within the expected time frame based on patient risk assessment

  • Acts as a resource to CTU and progressive care staff nurses, anesthesia, and perioperative team

  • Drives relationship with all team members caring for the CV surgery patient to achieve desired high quality patient outcomes and safe discharge plan

  • Serves as a resource to members of the health care team to facilitate patient care delivery by using an in-depth pathophysiological knowledge base


  • Energizes physicians and CV Service Line staff involvement in organizational clinical quality

  • Drives excellence in CORE measures through education, chart review and order preparation as appropriate

  • Models a culture of safety for the organization

  • Demonstrates a commitment to patient safety and as a leader, educates others regarding patient safety

  • Facilitates and implements systems which promote and maintain effective communication and collaboration with inter/intra hospital physicians, nurses and other members of the health care team

  • Maintains appropriate records and documentation of clinical and educational activities

  • Demonstrates ability to work effectively with patients, families, and with professional and supportive personnel who provide patient care

  • Recognizes and uses appropriate channels to communicate stressful patient care/performance situations that arise with physicians, families, and other staff members

  • Collaborates with the Medical Director of Cardiovascular Surgery and all CV Surgeons, Chief Medical Officer and Medical Director for the Cardiovascular Service Line regarding Quality Improvement activities. Contributes to development of new ideas to benefit patients, families, the CV Service Line, the Hospital, and/or staff

  • Acts as a role model and in selected cases, precepts advance practice nurses, i.e. nurse practitioner students (as appropriate)

  • Participants in activities necessary for annual competency review. Documents continuing competency in the areas of medical diagnoses, therapeutics, invasive procedures and management of the patient as we; as required certifications

  • Serves on departmental; and interdisciplinary committees as required

  • Performs other duties as determined by the Chief Medical Officer, Collaborative Physicians, Chief Nursing Officer, and Executive Director of the CV Service Line that are within the scope of practice of an APN


  • Acts as resource to Hospital staff and is available to provide educational programs which will improve clinical skills, enhance knowledge base and improve patient care

  • Maintains and enhances a current level of knowledge relative to professional practice, as well as continuing education requirements necessary for licensure and ongoing certification

  • Accepts self-responsibility for advancing skills and continuing education as evidenced by:

  • Sets realistic and measurable goals for individual developmental needs based on personal, peer, and supervisory evaluations of strengths and learning needs

  • Assesses own performance status in relation to this position description and progress toward achievement of professional goals on an annual basis

  • Seeks learning experiences/responsibilities to strengthen areas requiring development and to advance knowledge about other current developments in nursing


  • Prefer two years clinical experience and certification in his/her field of expertise

  • Current licensure as Registered Professional Nurse and certificate of recognition to practice as Advanced Practice Nurse as required by the State Board of Nursing (note: recognition by the State Board of Nursing as a Graduate Advanced Practice Nurse is acceptable until such time that successful completion of certificate has been achieved)

  • CV surgery and operating room experience required. Other candidates will be considered based on individual ability to understand the CV surgery patient and past education and work experience

  • Must be able to read, write and speak English
